Losing important documents is frustrating, especially something as important as your PHTA certificate and wallet card. We are here to help you and make the process as streamlined as possible. All you need to do is fill out one of the forms below.
Digital PHTA Replacement Certificate / Wallet Card Form
We’ll email you a digital version of your card as soon as we have all of your information and have verified your documents (usually within 2 business days). Your replacement card will have the same name and number as your previous card.
Hard Copy CPO Certificate / Wallet Card Form
PHTA is no longer automatically sending hard copies to CPO certification holders. If you would like to request a hard copy of your CPO Certificate / Wallet Card, please submit this form. PHTA will mail you a hard copy at no charge until January 1, 2025.
